Individualized Clinical Services

Axiom Intervention and Case Management Services (AICMS) can perform an extensive clinical mental health and/or substance abuse evaluation or assessment from the comfort of your loved one’s home/office.

 

The in-home assessment will determine the patient’s clinical and mental health/substance abuse diagnostic profile. A member from the AICMS team will help your family and loved one determine the best options for treatment, ongoing care, and longer-term treatment planning. 

They will meet with your loved one, assess their mental and psychological needs, and determine the appropriate level of care. This is based on the American Society of Addiction Medicine (ASAM) criteria, empirically derived research, and our team’s 42 years combined clinical experience in the field.
